Windscreen Insurance Claim Guide

To assist you with your windscreen insurance claim, please download and complete the claim form by clicking on the name of your insurer listed below. Once completed, email the form to info@glass-fix.com together with the following documents:

• Photographs of the damaged windscreen (refer to our photo guidelines)

• NRIC and Driving Licence of the policyholder and driver (if applicable)

• Certificate of Insurance

• Vehicle Log Card or LTA Access Code

• Preferred workshop location (Ubi or Toh Guan)
